July 2, 2020: CMS granted State final approval of their Statewide Transition Plan (STP).

Initial Approvals

July 9, 2018: CMS granted State Initial Approval Addendum of their Statewide Transition Plan (STP).

March 29, 2017: CMS granted State initial approval of their STP.

August 13, 2015: CMS notified State of Clarifications and/or Modifications required in order to receive Initial Approval (CMIA)
