Final Approvals

August 22, 2018: CMS granted State final approval of their Statewide Transition Plan (STP).

Initial Approvals

April 4, 2018: Addendum to CMIA Letter

December 28, 2016: CMS granted State initial approval of their STP.

July 9, 2015: CMS notified State of Clarifications and/or Modifications required in order to receive Initial Approval (CMIA)

Draft STP Additions by Date

September 14, 2015: Draft STP

January 26, 2015: Draft STP
